A questionnaire is a series of questions used to measure the performance of employees. Questions can be single choice, multiple choice, rating, or open-ended. Each participant (Reviewee and Reviewer) may be assigned a separate set of questionnaires to complete.

You may also create Questionnaires for each participant (Reviewee and Reviewers) and personalize your own Review Templates to match the structure of your company.

In this article, you'll learn how to:

Create a Questionnaire

Firstly, make sure you have access to Performance Settings. If not, please get in touch with your Admin to request it.

  1. Navigate to Performance > Settings

  2. Click on Questionnaires > + New Questionnaire

  3. Select a subject for the questionnaire and click Create

    • Reviewee only

    • Reviewer only

    • Pair of reviewer and reviewer questions (coming soon)

  4. Give it a name and describe when it can be used

  5. Click on + New Section to create a section name

  6. Click + New Question and:

    • Enter your first questions, give an example if necessary

    • Toggle the Required button if it is mandatory

    • Choose an Answer Type: Single, Multiple, Rating, or Text. You can also enable a Comment box and made it a requirement.

  7. Repeat until you are happy with your question list

  8. Click Save to finish

💡 Tips: You can create a new questionnaire by duplicating the existing ones.


View Questionnaire

  1. Navigate to Performance > Settings

  2. Click on Questionnaires

  3. Find the questionnaire that you want to preview, click ⋮ icon (More) > Preview


Edit Questionnaire

When editing a

  1. Navigate to Performance > Settings

  2. Click on Questionnaires

  3. Find the questionnaire that you want to preview, click ⋮ icon (More) > Edit

  4. Edit the content

  5. Once you're happy with it, click Save to apply changes

⛔️ Caution: Before deleting a section, make sure you move all questions to another section, or all contents will be deleted.

📌 Note:

  • You CANNOT adjust the subject (i.e. Reviewee and Reviewer) of the questionnaire.

  • You can drag and drop questions to rearrange your question list

  • Sections can be organized by clicking on : (More) > Move Up/Move down

Delete a Questionnaire

📌 Note: You can only delete questionnaires that are NOT in use.

  1. Navigate to Performance > Settings

  2. Click on Questionnaires

  3. Find the questionnaire that you want to delete, click ⋮ icon (More) > Delete
